Joseph Carroll Pilot Crash, Miami, FL – Amazon Cargo Plane Crash Pilot Injured

Joseph Carroll, 55, has been identified as one of the two pilots aboard an Amazon cargo plane that overran a runway at Miami International Airport on September 6, 2026, in a crash that killed five people.

Carroll was the captain of the Boeing cargo flight operated by 21 Air, according to information cited by CBS News from the Miami-Dade Sheriff’s Office, LinkedIn profiles and the Federal Aviation Administration’s pilot database. Jaime Felipe Silva Molina, 37, was identified as the flight’s first officer.

The Prime Air Flight 7598 aircraft had departed from Luis Muñoz Marín International Airport in San Juan, Puerto Rico, before overrunning the runway while landing in Miami. The plane struck airport navigation equipment before colliding with a cleaning crew van carrying seven people.

The aircraft then broke through a fence and struck a Toyota SUV traveling outside the airport before crossing another fence and coming to a stop near parked Tesla robotaxis. All five people who died were occupants of the two vehicles struck by the aircraft.

A witness, Abraham Altamar, told CBS News that one of the pilots emerged from the cockpit visibly shaken and injured following the crash. Altamar said the pilot asked to use his phone to call his wife and told her he was safe and out of the aircraft. The pilot’s wife later recalled hearing him say, “My career is over.” It was not immediately clear whether the pilot was Carroll or Silva Molina.

Both pilots were treated for injuries and later released, according to National Transportation Safety Board Chair Jennifer Homendy.

The NTSB is investigating the crash. The aircraft’s cockpit voice and flight data recorders have been recovered and sent to the agency’s headquarters in Washington, D.C., for analysis.

Investigators are also examining whether runway safety measures, including an Engineered Materials Arresting System, could have helped prevent or reduce the severity of the crash.

The investigation remains ongoing, and officials have not yet determined the cause of the runway overrun.
